PLEASE NOTE NEW POLICY FOR NEWPORT HEART BEGINNING JANUARY 2023
Effective January 1, 2023, Newport Heart will be instituting an administration fee to cover the cost of all documents and/or forms that are requested to be completed by our office staff or physicians.
This includes surgery clearance letters, disability forms, travel forms, dental clearances, insurance letters, FAA letters, Jury letters, DMV forms, and any other such forms requiring medical documentation.
Patients may opt to pay an annual administration of $75 per year to cover ALL documents requested during a 12-month interval,
OR
Patients may choose to pay a $50 fee payable at the time of the requested service for each individual document that is needed throughout the year, to a maximum amount of $100 per year.
Fees will be billed to your credit card and are due upon receipt of the request.
